In addition to the Online Registration Form and payment, families will need to complete some additional forms in order to attend South Sound Day Camp.

All participants must complete the COVID Waiver Form.
 

All participants will need to complete Section 1 of the Health History Form. (If a youth needs medication administered, the corresponding section will need to be completed.)
